About this policy

I am Brita Graser, M.Ost, a registered osteopath providing osteopathy, ScarWork and clinical Pilates in Queenstown. I handle all personal and health information in accordance with the Privacy Act 2020 and the Health Information Privacy Code 2020.

This policy sets out what information I collect, how it is used and stored, and your rights. It applies to my clinical practice and this website.

What information I collect

I collect only the information required to provide safe and appropriate care, including:

•        Contact details: name, address, phone number, email address and date of birth

•        Health information: medical history, symptoms, medications, previous injuries and surgeries, and general health

•        Clinical records: assessment findings, treatment notes and treatment plans

•        ACC and health insurance details, where relevant

•        Payment records

Information is collected directly from you where possible. With your consent, I may also obtain information from other health providers involved in your care.

How I use your information

Your information is used to:

•        Assess, treat and manage your care

•        Lodge and manage ACC and health insurance claims

•        Send appointment confirmations and reminders

•        Process payments and maintain financial records

•        Meet legal and professional obligations

Where you have subscribed to my newsletter, your email address is also used for this purpose. You may unsubscribe at any time.

Who I share your information with

Your information is confidential. It is disclosed only where necessary for your care, with your consent, or where required or permitted by law. This may include disclosure to:

•        ACC, in relation to a claim

•        Your health insurer, at your request

•        Other health providers involved in your care, with your consent

•        Service providers supporting the operation of my practice, such as practice management and ACC claiming software, solely to provide that service

Personal information is never sold or rented to third parties.

In limited circumstances, information may be disclosed without consent where permitted by law, for example to prevent a serious threat to health or safety.

How I store and protect your information

Records are stored electronically in Cliniko, a secure cloud-based practice management system, with access restricted to me. Some service providers may store data on secure servers outside New Zealand, subject to protections comparable to New Zealand law.

In accordance with the Health (Retention of Health Information) Regulations 1996, health records are retained for a minimum of 10 years after your last appointment and then securely deleted.

Any privacy breach likely to cause serious harm will be notified to you and to the Office of the Privacy Commissioner, as required by law.

Information submitted through the website or the online booking system (Cliniko) is used only to respond to your enquiry and manage your appointments.

This website may link to external sites, whose privacy practices are not covered by this policy.

Your rights and how to contact me

You are entitled to request access to the information held about you and to request its correction. Requests will be answered within 20 working days, usually at no charge.
